Forms

Once you have accepted a co-op offer, fill out the following forms and return to the Career Center with a copy of your offer letter and job description:

Co-op Work Agreement Form – This confirms your co-op offer and provides Clarkson with the company name and contact information, as well as your address and contact information, while you are on co-op.

Co-op Academic Plan – Use this form to create a schedule for completing your degree, including the time spent on co-op. Have this approved by your advisor, a Student Administrative Services representative and return to the career center for final signature.

Student Participation Agreement – This document outlines the rights and responsibilities of a student participating in the co-op program.

Co-op Assumption of Risk Form – This agreement outlines responsibilities for potential injuries and health and liability insurance responsibilities.

Co-op Expense Form – This is a form to use while you are in your co-op position. Clarkson does not reimburse these expenses, but this helps document your co-op position for financial aid purposes.  Return to your SAS representative when you return.

While on Co-op, please be sure that you and your supervisor fill out these evaluations.  Initial evaluations should be completed after approximately 1 month of work and the final evaluation should be completed your last week on the job.

Student Initial Evaluation Form – This is an opportunity for you to evaluate your co-op experience. You complete this form one month after your co-op job begins.

Employer Initial Evaluation Form – With this form, a co-op employer provides Clarkson with feedback on the student’s performance. This form is completed one month after the co-op begins.

Student Final Evaluation Form – Complete this final evaluation of your experience when your co-op has ended.

Employer Final Evaluation Form – This is the employer’s final evaluation of the co-op student’s performance.
